Online Request Forms

• Book Renewal
  Faculty, staff, and students may renew books online from the CUNYplus catalog by logging into My Account and entering their LIBRARY BARCODE NUMBER and password.

• Interlibrary Loan
  Faculty, staff, and students may obtain materials not available at the CSI Library by submitting a request through Interlibrary Loan (ILL).

• Library Instruction
  Faculty may request a general library lecture or a lecture on a specific topic for a class by first calling Prof. Linda Roccos at 718.982.4004 to locate a suitable time slot, and by then filling out the Library Instruction Request Form and faxing it to 718.982.4002. Classes are not generally given the last four weeks of the semester.

• New Book/Media Acquisition Request
  Books or materials may be suggested for new acquisition by filling out the New Book Acquisition Request Form.
